How To Register

 

We welcome new patients living within our catchment area who wish to register with us. Patients who do not reside within our area but who live with a family member who is already a patient at the practice (due to historical boundary changes) may also register. If you would like to register, please check you live within our practice area.

 

Please bring the following to the practice after 3.00pm (if at all possible) on any day except Thursday:

  • your NHS medical card (if you do not have an NHS medical card, you will need to complete form GMS1)
  • proof of your address (eg a utility bill or bank statement no older than three months)
  • photographic identification (eg passport, driving license). If you have trouble with this, please let us know.
  • your child's Red Book if you are registering a child aged six years or under (we will take a copy of immunisation history)

If you are a family registering together, we need only to see proof of address for one adult family member although you will need to complete form GMS1 for all family members who do not have NHS cards. If you have recently had a baby, please register the baby's birth and bring with you form FP58 which you will be given at the time of birth registration. You do not need to bring any other documents but it would help us if you are able to pre-register the baby beforehand. If you have difficulty providing proof of address - for example if you are homeless - we will do our best to help you and suggest that you speak to one of the managers in the practice.

 

If you are currently registered with another local practice and wish to register with us because you are not happy with your practice, we will be happy to register you but ask that you attend a pre-registration appointment with a doctor to discuss this. Please call us on 020 8423 8423 to arrange this. You may wish to pre-register and bring along the necessary documents ready for registration when you attend for this appointment.

 

When you attend the practice with your documents, a receptionist will check your details and register you straight away. We like all our new patients to attend for a new patient health check with the nurse and this will be arranged at a time convenient for you when you come to register. We would be grateful if you would bring a completed copy of our new patient health check form to this appointment.
